At CrossBorder Pickups, we try our best to ensure that your shipping and receiving needs are met in a timely and efficient manner. As part of this, we make sure to fully comply with all federal requirements set out to us. One such requirement is the completion of form 1583 for any mail and deliveries through the U.S. Postal Service.
United States Postal Service requires that any agent that receives mail on behalf of a recipient must have written consent of that recipient. We are required to comply with this request and have you, the customer, sign form 1583 in our service location and provide 2 forms of valid ID.
At our service locations, we will provide you with a pre-populated form 1583 to save you some time. All you need to do is sign and provide 2 pieces of ID to our Customer Service Representative. If you are unable to come into any of our two locations, you can simply email [email protected] to request for your form. Simply sign and attach with a copy of the IDs and email us back.
It is imperative that you ensure you are presenting ID which complies with the requirements.
Once completed, we can continue to receive mail on your behalf uninterrupted. This form only needs to be completed once unless there is a change to your information.
Unfortunately, USPS will not allow any agent to receive your mail without this requirement being met.

ID Requirements
The following are acceptable IDs as stated on form 1583
Valid driver’s license or non-driver’s identification card
– Recognized corporate ID (work ID)
– Health card
– Student ID
– Passport, alien registration card (aka “green card”), certificate of naturalization
– The current lease, mortgage, or deed of trust
– Voter or vehicle registration card
– Home, health, or vehicle insurance policy
Please note that social security cards, credit cards, and birth certificates are NOT acceptable forms of identification.
Your primary ID MUST be Photo ID such as drivers license or health card (a passport cannot be accepted as photo ID).
The secondary piece of ID must have your name and address (eg. a bill, banking statement, lease or mortgage letter, business registration, business invoice, etc.)
The information provided on the ID must match the name and address that CrossBorder Pickups has on file.
If you have a business registered as the address on file, the 1st photo ID can be a driver’s license and the 2nd piece of identification needs to match the business address.
